Although it hasn’t been discussed openly, many caregivers suffer sexual harassment by people having dementia in nursing care facilities or home care service.
Our organization occasionally has opportunity to hold caregiver’s meetings, where young female caregivers often share their troubles.
They talk about unacceptable experiences, sometimes in tears.
Sexual harassment by elderly people is attributed to dementia.
A doctor says this kind of wrong acts are often committed by those who have lived decent lives, such as retired school teachers or civil servants, on the rebound of strong sexual inhibition in their youth.
Also single males who have lived alone for a long time after losing their wives tend to show this kind of behaviors.
They become unable to help doing it probably because of loneliness, when their inhibition is undermined by dementia.
A female says she hates her husband’s father, as he told her to get into his bed together when she was taking care of him.
Giving words of caution might be difficult, especially when the harasser is her father-in-law.
Some wives hesitate to tell their husbands about it, being afraid of having the relationship get awkward.
The harasser, on the other hand, may have mistaken his son’s wife for his own wife.
Today, increasing number of female users of care facilities are also reported to commit the misconduct to male caregivers.
They act wrong, like touching the crotch area of caregivers, demanding kiss or hug, or showing stalking behaviors with strong attachment to specific caretakers.
In many cases, their strong jealousy causes troubles with other females.
However, these cases are less focused, because males are considered to have less feeling of disgust or rejection.
Of course, there are also LGBT cases.
In addition, there are problems between residents.
Usually, rooms are not equipped with locks.
A caregiver found a pair of male and female residents sleeping naked in a same bed, when going the rounds at night.
Some care professionals hesitate to report to families of the residents who are hard to be cared of at their own homes because of severe dementia symptoms.
Actually, uncovering everything doesn’t necessarily make everyone happy.
That’s why many care professionals are suffering alone.
It is very difficult to deal with these troubles, as people with dementia don’t really know what they are doing.
And this kind of problem is a delicate issue for everyone that you don’t feel like make public.
However, it can’t be left unaddressed.
Some people suspect that sexual deviancy might be one of the reasons for group infections occurred in a number of care facilities.
Let’s think about how we can protect dignity of people with dementia without giving up respecting caregivers.
Dementia causes disinhibition, as well as difficulty in social cognition.
This can lead to wrong actions by people with the disease, such as saying obscene words, exposing genitals or pressing for sexual acts, which badly confuse their family members.
These acts can be a reason for denial of using professional caregiving service.
Caregivers who are faced to sexual behaviors by patients are very surprised at first and develop strong aversion, as most of them didn’t imagine that their care receivers had sexual desire.
Here are findings of a survey conducted by Japan Society of Sexual Science.
Most of singles in their 60s or 70s have active sexual interest to those of the opposite sex.
Half or more of married males at the age of 60 or older want to have sexual affair or caress with their wives.
On the other hand, male’s sexual function begins to declines at his late 50s.
Most of care receivers are not able to have sexual activity anymore.
However, that doesn’t mean they don’t have sexual interest.
It is natural to have sexual interest after getting old enough to need caregiving.
We need to recognize that we never lose our sexuality until the end.
We can easily react emotionally to sexual behaviors by aged people with dementia.
But we need to consider why they do it, how they feel or what the background is to deal with those cases.
Let’s start by thinking about feelings of anxiety, loneliness or detachment that they could have, which might be the background.
Here are findings of a survey to care giving professionals.
42% have been sexually harassed.
56% have experienced physical and/or mental violence.
That means about half of professional caregivers are victims of sexual harassment or violence.
These caregivers might quit the job when they can no longer stand it, which leads to labor shortage in this field.
Those who need nursing care, including many good and decent people, will have less access to the service.
Leaving the problem untouched brings nothing to users or caregivers.
What you should do at first when sexually harassed is to refuse calmly.
Emotional reaction could make them simply feel that they are disliked.
Or some might enjoy seeing your response and escalate their bad behaviors.
The most important thing is to act calm.
A doctor talks about how a female experienced caregiver deals with the problem.
When she had touched her breasts, she reacted calmly without panic saying ‘Naughty boy, I have big tits since I was a girl. But I’ll tell your wife if you do it again.’
Then she took the man’s hand and said, ‘Now I’ll bring you a cup of tea.’
We respect caregiver’s dignity as much as care receiver’s.
This doctor said the caregiver’s response was ‘brilliant’, but we don’t want it to be the conclusion of the story.
Let’s explore the best options for both sides.
One way to put a stop is turning a care receiver’s attention to something else on TV or out of the window.
Maybe you can say ‘Hey, what is that?’ and distract the person’s attention to something other than yourself.
If you are taking care of somebody in your family, you have an option to ask for help when you have another family member.
Female care receivers are said to have stronger uncomfortable feelings when taken care of by someone of the opposite sex.
Basically, we shouldn’t allow gender discrimination.
But we should build a system to protect dignity of elderly people, where care receivers can decide the sex of their caregivers.
Many people think male and female residents should be live in different wards, and in case that is impossible, they should live on different floors and rooms should be locked during the night.
Some people point out the need for sexual service for the elderly or those having dementia.
Once a sex toy company announced to collaborate with a care facility for promoting masturbation to reduce sexual deviancy.
It seems that was not a good solution, as nothing reported after the launch.
A doctor working at a care facility says, ‘I know it is not comfortable for some people, but the best realistic option is medication.’
Among various antidementia drugs, Memary has efficacy against abnormal sexuality.
If it doesn’t work well, another option would be Gramalil that is effective against vascular dementia patient’s excitation, although it has side effects including disturbance in gait.
However, using a chemical drug that has impacts on cranial nerves may sound unacceptable for many people.

Putting aside whether medication is acceptable or not, we think one of the reasons for the sexual behaviors is loneliness.
If you have a family member living in a care facility, go to see the person just once a week or so and have a bit of conversation.

According to a new report looking at Alzheimer’s disease and dementia worldwide, around 75 per cent of all dementia cases are undiagnosed — and due to advancements in treatments and testing, there could be a “tsunami of demand” for diagnosis in the next few years.
The World Alzheimer Report, released this week by Alzheimer’s Disease International (ADI), found that the issue was more pronounced in low-income countries, where up to 90 per cent of dementia cases have no medical diagnosis.
Currently half a million Canadians live with dementia, with that number expected to increase to almost one million by 2030. Canada, which is classified as a high-income country, is estimated to have 60 per cent of its dementia cases undiagnosed.

At home and abroad, Dr. Kazuo Hasegawa, now 92, has enormously contributed to establishment of the groundwork for diagnosis of dementia.
In 2017, at the age of 88, he disclosed his disease.
After spending many years studying dementia, Dr. Hasegawa himself finally developed the disease.
He decided to make it public, because he wanted to let people know that anybody can have dementia.
And also, he wanted to make society more open to the disease.
Hoping to make it easier for those living with dementia to talk about the disease openly, he published a book titled I’m Enjoying my Spiritual Life with Dementia.
In the book Dr. Hasegawa shares 100 things he found.
61. Nursing care for people living with dementia is not limited to assistance for eating, excreting or bathing.
62. Tom Kitwood listed five psychological element that those living with dementia want: Comfort, attachment, occupation, inclusion and identity.
63. A shift in the place to live or change of the responsible caregiver may lose feeling of security or certainty that those with dementia have.
64. Engagement in something must be the way to live a life with personal meaning, which is opposite to boredom, inaction or apathy.
65. The true self or life story means the identity that exists beyond your living condition or the role you are playing.
66. Person-centered care is not a way of caregiving focusing on a family member’s or caregiver’s view, but a way to respect a specific view of every person living with dementia
67. The attitude of person-centered care is to appreciate who he is, instead of what he can do, through which a caregiver can walk together, share joy and learn something in life.
68. The attitude opposite to person-centered care is called malignant social psychology, which includes outpacing, disempowerment, disruption, imposition and ignoring.
69. There is differences in the sense of the world between those living with and without dementia, but caregivers often expect care-receivers to fit the world for those without dementia. Accept care-receiver’s sense of world.
70. Exploring how to deal with a problem together with a care receiver and offer necessary help, and taking care of everything are two different attitudes.
