In early November, China Food and Drug Administration (CFDA) was reported to give conditional approval to Oligomannate (GV-971),
a new anti-dementia drug developed by the Shanghai Institute of Materia Medica and the Ocean University of China.

Here on this issue, we present our view on this drug.

We presume the background of the research by Gen Meiyu.
Her team on the drug targeting the brain-gut axis is a hypothesis
that changes in the balance of intestinal flora cause accumulation of phenylalanine and isoleucine in the peripheral blood,
which induces neuro inflammation and cognitive dysfunctions.

To understand the mechanism of the brain-gut axis, we need to know the interaction between intestinal flora and the central nervous system.

The brain and intestine have a close two-way relationship through autonomic nerves,
hormones, cytokine and so on, which is called brain-gut interaction or brain-gut axis.

Recent studies showed the intestinal flora substantially affects brain’s functions and human behaviors besides health of the digestive canal.

More specifically, the intestinal flora is involved with brain development, stress reaction, anxiety, depression and other functions or actions of the central nervous system.

Also in the field of the research on Alzheimer’s disease, disbalance of intestinal flora is said to be closely related to the disease.

The new drug Oligomannate is basically a compound of oligosaccharide and sodium.

In this study, researchers picked up marine oligosaccharide and investigated effective action of Na-AO derived by enzymatic decomposition of alginate sodium salt.

The result of experiments on rats suggests Na-AO administration can develop PPARα activating action and reduce apoA-1.

However, extrapolation to human body still needs work due to substantial requisite dosing amount.

Oligosaccharide is well known to have intestinal regulating effect or anti-cavity effect, along with proabsorptive effect recently found,
but very little analysis has reported lipid metabolism improving effect.

Also, in contrast to terrestrial oligosaccharide, marine oligosaccharide has limited number of reports due to difficulty on production.

Further researches are expected to find Na-AO’s physiological effects and wide range of application as food materials supporting lifestyle diseases prevention.

As the study above, oligosaccharide or sodium has been tested almost for 20 years
by Japanese medical society on their physiological effects for lifestyle disease prevention.

Those substances are known to be safe to human body.

Fortunately, the result of the study by Gen Meiyu’s team shows the new drug significantly improved cognitive dysfunction of mild and moderate Alzheimer’s patients.

However, what patients, family members or others suffer from the most are symptoms of severe cognitive impairment.

Severe core symptom is accompanied by various BPSDs such as abusive language, violence, excitation, depression, sleep disorder, day-night reversal, hallucination, delusion, delirium, wandering, coprophilia or incontinence.

Until now, four types of medicine have been used for curative or preventive purpose: donepezil (Aricept), galantamine (Reminyl), rivastigmine (Exelon) and memantine (Memary).

Unfortunately, none of them could suppress serious BPSDs as above,
and finally French authority decided to exclude them from insurance coverage due to insufficient effectiveness.

The new drug Oligomannate (GV-971) seems to be less likely to entail adverse side effects which sometimes worsen BPSD as seen in the four drugs.

We can say it is unlikely to be harmful to mild or moderate Alzheimer’s patients.

 

However, we presume the drug can’t necessarily show great result on the symptoms mentioned above.

WHO’s Magna Carta for World Health give a broad definition to health as “a state of complete physical, mental and social well-being”.

In light of this definition, our association pursue physiological approach aiming to help human body’s functions recover to the state where it should be.

We believe the key to recover the health is regulating bloodstream to suitable state for human bodies, which can differ a lot from each other, based on the outcomes of researches on restoration of health.

Our hope in 2019 is to see a community achieve zero dementia by recovering cognitive functions.

We believe taking reishi in proper manner is crucial for normalizing microcirculation, which highly contributes to the recovery.

We hope this information reaches as many people as possible to stop tragedies such as caregiver murders or fatal accidents by the elderly.

We are very happy to see our action to achieve dementia free society in a fundamental way based on good understanding of human body shared and disseminated across the world.

Our association has conducted physiological researches to prove cognitive functions can recover, as well as offering a wide range of awareness campaigns.
