Identification of acceleration factor of cognitive decline in mild cognitive impairment

 

Tokyo University graduate school medicine system graduate course professor Mr. Takashi Iwatsubo and group announced in the American Science journal as the title of “ identification of acceleration factor of cognitive decline in mild cognitive impairment “.

J-ANDI study states that female mild cognitive impairment worsens than male, the reasons are chronic kidney disease or longtime hypertension arteriosclerosis will affect the progress of cognitive impairment.

This research presentation is very meaningful in order for everyone to understand the activities of our association.

J-ADNI is Alzheimer’s disease study project led by Japanese Ministry of Health, Labour and Welfare, NEDO etc. in 2007.

J-ADNI is abbreviation of Japanese Alzheimer’s Disease Neuroimaging Initiative ( leading research of Alzheimer’s Disease brain imaging diagnostic method in Japan. ) It started after the example of American Alzheimer’s disease study (ADNI).

Mr. Takeshi Iwatsubo ( 1960 ~ ) is the Japanese neuropathologist. Professor of graduate school medicine system graduate course of Tokyo University. Leading person in Alzheimer’s disease, dementia study in Japan. Representative of J-ADNI.

Apolipoprotein E ( ApoE ) is very important gene which is related to outbreak of Alzheimer’s disease.
ApoE gene has three subtypes and those are e2, e3, and e4.
Among those, two comes from parents.
It is said that persons who have e4 are higher risk of developing Alzheimer’s disease than those who have not.
In general, in case of having one e4, the risk of developing is three times and having two e4s , the risk becomes ten times.
But all the people who have ApoE e4 have not developed dementia.
But those who have e4 recognize the risk and it is necessary to improve life style customs not to become dementia.

 


 

In “ Identification of acceleration factor of cognitive decline in mild cognitive impairment “, they studied various elements which are related to lower the cognitive functions for three years, from the various angles such as examination of cerebrospinal fluid, examination for amyloid PET, and ApoE genetic screening, etc.

Especially prevalence of ApoE e4 which is said the strongest risk factor of Alzheimer’s disease is focused on, they found out the progress is influenced by sexual difference, history of education rather than the generic background.

 

As for difference in sex, females showed quicker decline in cognitive function than males.

Males with longer educational history, who have university or higher level degrees, showed milder decline.

Such difference by education was not found among females.

Further research by sex showed females with higher grades of chronic kidney disease suffer more rapid decline.

The report estimated that dysfunctions in brain’s smaller vessels caused by longitudinal high blood pressure or arterial sclerosis affected progress of cognitive dysfunction.

 


 

The study showed chronic kidney disease was significantly responsible for boosting cognitive dysfunction among Japanese (especially females), so knowing more about functions of kidney or the cause of its disease to prevent the disease must be a brake for controlling MCI from transitioning to dementia.

Kidneys are fist-sized organs, located both right and left on the back side around loins. They constantly receive about one fifth of the whole blood pumped from the heart, and clear out excessive water, waste products etc. They clean dirty blood and discharge the waste as urine.

 

In kidney disease, hypertonia becomes a risk factor. If the function of kidney worsens, it cannot excrete extra salt and water, then the blood amount increases and the blood pressure rises. Once the blood pressure goes high, the burden to the kidney increases, then the function of kidney lowers, it becomes vicious circle. Therefore, in order to protect the function of kidney it is important to control blood pressure. In controlling the blood pressure, it is important the microcirculation since the kidney is consist of enormous fine blood vessels. ( peripheral blood vessel ).

 


 

Dementia improved support Association of Japan, before the announcement of J-ADNI study, thought that it is important for the improvement of dementia to prevent the life style related disease by functioning microenvironment normal since dementia is closely related life style related disease.
